Language Assistance

Your Rights as a Client in Wisconsin

Under Wisconsin Statute 51.61 and DHS 94, you have the right to be treated with dignity and respect, receive services in a safe environment free from abuse and exploitation, participate in your own treatment planning, refuse treatment, have your records kept confidential, file a grievance without retaliation, and access your own records.

How to File a Discrimination Complaint

HHS Office for Civil Rights: File online at ocrportal.hhs.gov, by phone at 1-800-368-1019, or by mail at 200 Independence Ave SW, Washington D.C. 20201. Complaints must be filed within 180 days of the alleged act.

Wisconsin DHS: File with the Division of Quality Assurance at dhsdqamentalhealthandsubstanceusecertification@wisconsin.gov.
